Evergreen Cottages Volunteer Acknowledgment - EGC-VOL-2026-10-28

I confirm that the information I provide is accurate and that I am authorized to submit it. I understand that this is an application for an unpaid volunteer role. Submission does not guarantee acceptance, employment, placement, scheduled activities or a minimum number of service hours.

I will not begin volunteering until Evergreen Cottages approves my application and I complete any required orientation, verification and other prerequisites. I will register for available opportunities through the Volunteer Portal and follow the directions of authorized staff. Evergreen Cottages may determine suitable assignments, limit participation, decline an application, or suspend or end volunteer access or participation, subject to applicable law and its program requirements.

I will follow applicable safety, conduct and resident privacy requirements; respect residents, families, staff and other volunteers; and keep confidential information obtained during volunteering private. I will not photograph, record, publish or disclose resident information without the required authorization. I will not provide services outside my approved assignment or represent that I am authorized to act for Evergreen Cottages. I will promptly report safety concerns or incidents to the supervising staff member.

I understand that submitted service hours require staff verification and approval, and that certificates reflect approved hours only. I will report my attendance and hours honestly.

I have read and agree to the website Terms and Conditions and acknowledge the Privacy Policy. Evergreen Cottages may use my application and contact information to administer my application and volunteer participation as described in those policies. Optional email and SMS program communications remain subject to my separate consent choices; this acknowledgment does not enroll me in either program.

By checking the acknowledgment box and typing my full name, I intend to sign this acknowledgment electronically and confirm my agreement to it.
